Built in a strategic position in the Grana River valley and a transit point along the ancient Roman road that linked Asti and Casale, the "castrum" of Altavilla is mentioned in the papal bull of Pope Eugene III to the Bishop of Asti (1153). The Parish Church of San Giulio houses several works by Moncalvo. The church was consecrated in 1630 and houses the Last Supper by Caccia and the Wedding at Cana by Musso.

Altavilla is home to the Steam Tramway Museum, an integral part of the Ecomuseum of Basso Monferrato Astigiano, which displays several recovered and restored collectibles.

The Grappa Museum displays the original 19th-century steam still and other old distillation and bottling equipment.
