The building stands on the edge of the hamlet of Montale Celli.
The salient façade, mostly in exposed brick, is divided into three unequal sections by pilasters that cross it from the base to the eaves. The majestic splayed portal has two pairs of slender columns supporting a pointed-arch lunette resting on corbels and containing a high relief with a polylobed motif. It is flanked on each side by side entrances, also enclosed in projecting cornices with pointed lunettes. A rose window opens at the base of the tympanum, while two acroteria define its lateral extremities. The eaves are punctuated by a succession of small suspended arches. The bell tower has a square cross-section and is integrated into the structure, on the right side, near the apse. Entirely adorned with exposed brickwork, it has a tower with a tall pointed arch that frames the clock at the top. The bell tower features single-lancet windows on each side and is covered by a small octagonal prism roof. The central nave has a double-pitched roof, while the side naves have a single-pitched roof with a tiled roof. The church has a basilica plan with three naves. The presbytery ends with a curvilinear apse. The central nave has a barrel vault, while the side naves have ribbed vaults. The baptismal font is on the right side, near the entrance. The floor is in Genoese marble chips in the nave, and marble in the presbytery.
