It is a scattered municipality, as the municipal territory is a "federation of hamlets." The municipal seat is located in S. Lorenzo, one of the 13 hamlets.
The Groppo castrum is cited as belonging to the Malaspina family of Olivola in the partition of 1275 and was subsequently destroyed by the people of Voghera in 1398. Currently, no trace of it remains and it should not be confused with the current (private) castle of Pozzol Groppo.
During the Napoleonic era, the town was an autonomous municipality. The municipality of Groppo was abolished in 1928 and merged with Pozzolo del Groppo, hence the name Pozzol Groppo.
In 1449, the town of Pozzolo del Groppo was recognized as being under Tortona control.
Tortona authority vanished in 1530 when Marquis Cesare Malaspina of Godiasco seized the castle.
It remained a fiefdom of the Malaspina family until 1797.
It appears with the name Puzolum in 1176 and as Pozolio de Gropo in 1202, derived from the Latin puteum, meaning well, with the diminutive suffix -olus, meaning small well, likely referring to a spring or watering hole. The term Groppo refers to the hamlet of Biagasco, also known as Groppo, meaning hill, which was a separate locality from Pozzolo del Groppo until 1928, when they became a single municipality.
